Abstract : Background: Fatty acid desaturase 1 gene encoding delta-5 desaturase enzyme is one of the candidate genes for studying metabolic risk factors. Delta-5 desaturase activity plays a crucial role in the pathogenesis of metabolic syndrome because reduction in delta-5 desaturase activity may lead to abnormal long chain polyunsaturated fatty acids metabolism. \r\n\r\nObjectives: The purpose was to study fatty acid desaturase 1 gene polymorphism (rs174546) and delta-5 desaturase activity which is estimated by serum fatty acids ratio in metabolic syndrome.\r\n\r\nMethods: This cross-sectional analytical study included 84 subjects with metabolic syndrome and 84 subjects without metabolic syndrome. Metabolic syndrome was identified by International Diabetes Federation metabolic syndrome criteria. Genetic analysis of fatty acid desaturase 1 polymorphism (rs174546) was done by Sanger’s sequencing. Serum omega-3 fatty acids (alpha-linolenic acid and eicosapentaenoic acid) and omega-6 fatty acids (linoleic acid, dihomo-γ-linolenic acid and arachidonic acid) levels were determined by gas chromatography-mass spectrometry. Delta-5 desaturase activity was estimated by the ratio of serum arachidonic acid level to dihomo-γ-linolenic acid level. \r\n\r\nResults: The frequency of T/T, C/T and C/C genotypes of fatty acid desaturase 1 polymorphism (rs174546) were found in 14.3%, 36.9% and 48.8% in the subjects with metabolic syndrome and 14.3%, 47.6% and 38.1% in the subjects without metabolic syndrome. There was no significant difference in genotype distributions of this polymorphism (rs174546) between metabolic syndrome and without metabolic syndrome (p<0.2). The mean fasting blood sugar level was significantly higher in minor variants of fatty acid desaturase 1 polymorphism (rs174546) in metabolic syndrome (p<0.05). The mean estimated delta-5 desaturase activity was significantly lower in subjects with metabolic syndrome (p<0.001). The mean values of linoleic acid, dihomo-γ-linolenic acid and alpha-linolenic acid were significantly higher in T/T than C/T and C/C genotypes in metabolic syndrome (p<0.05). The mean estimated delta-5 desaturase activity was significantly lower in T/T genotype than C/T and C/C genotypes in metabolic syndrome (p<0.001). \r\n\r\nConclusion: The delta-5 desaturase activity was associated with fatty acid desaturase 1 polymorphism (rs174546) in metabolic syndrome. Mean estimated delta-5 desaturase activity was found to be lower in T/T genotype of subjects with metabolic syndrome in the present study. Lower delta-5 desaturase activity resulted in lower serum long chain omega-3 fatty acid levels which might be one of the contributory factors to metabolic disturbances, especially dyslipidemia. Present study can explore one of the mechanisms underlying dyslipidemia due to reduced delta-5 desaturase activity as a consequence of fatty acid desaturase 1 polymorphism in metabolic syndrome. This study suggested that subjects with fatty acid desaturase 1 polymorphism should be taken into consideration in the management of dyslipidemia in metabolic syndrome.\r\n
