Title of Research : Clinico-Etiological profile of Acute Encephalitis Syndrome at Yangon General Hospital, Myanmar

Field of Research : Neurology

Preliminary Research Design : Descriptive Study

Preliminary Research Type : Applied

Preliminary Justification : Diagnosis of acute viral encephalitis is challenging especially in resource-limited countries and Myanmar is not an exception. It is fundamental to find out the common possible etiological pathogens in order to reduce morbidity and mortality as consequences of viral encephalitis. The morbidity and mortality from viral encephalitis in Myanmar is high according to Hospital based data. There is no accurate country-wide data on incidence, mortality rate and causal organisms of viral encephalitis in Myanmar because there are only limited studies. It is essential to know the infectious pathogens to give specific therapy and to take measures for prevention. Therefore, there is an urgent need to conduct focused research to identify the etiological agents in acute viral encephalitis cases in order to give better management of patients, to help reduce the morbidity and mortality, and to take measures for better prevention and control strategies. This study will be a fundamental research in predicting the common pathogens of viral encephalitis in Myanmar as study area is a tertiary center receiving patients from different parts of the country. Based on the results of this study, infectious agents causing acute encephalitis syndrome in Myanmar will be known and patients can be effectively treated with specific antimicrobial therapy and subsequently reduce mortality and improve outcome. In addition, future research on viral encephalitis with the focus on epidemiology, clinical outcome and preventive strategies can be conducted.

Preliminary Aim : To determine the infectious etiology and clinical profile of patients with acute encephalitis syndrome

Preliminary Objectives : 1. To describe the demographic characteristics of patients with acute encephalitis syndrome 2. To define the clinical characteristics of acute encephalitis syndrome 3. To determine the infectious etiologies of acute encephalitis syndrome

Preliminary Sample Size : All consecutive patients with acute encephalitis admitted at Neuromedical department of Yangon General Hospital who fulfill inclusion criteria will be recruited within the study period. Inclusion Criteria All the patients older than 12 years admitted to Neuromedical department with (1) Acute onset of fever within 5 days AND (2) Change in mental status (including symptoms such as confusion, disorientation, coma, or inability to talk) AND/OR (3) New onset of seizures was included in this study. Exclusion criteria Patients with different final diagnoses (e.g., epilepsy, tuberculosis meningitis, brain tumor, cerebral malaria, stroke, or metabolic encephalopathy) were excluded from the study.

Preliminary Study Duration : October 2018 to September 2019

Preliminary Study Area : Neuromedical department, Yangon General Hospital and National Health Laboratory

Preliminary Study Method : Descriptive, cross-sectional study

Preliminary Research Outcome : : Demographic Information, Clinical Information, CT/MRI (Brain), EEG, JBE serology (IgM), Lumbar puncture and CSF study

Preliminary Research Finding : Categorical variables will be summarized in proportion, and numerical variables were described in mean (SD) or median as appropriate.

Preliminary Progress Report : Out of 40 encephalitis cases collected up to now, 2 cases were measles encephalitis, 1 case was varicella zoster encephalitis, 1 case was dengue encephalitis and 1 case was Herpes simplex encephalitis according to clinical history , film array and serum
